At this clinic, learn how to prepare for disability and end of life so that your wishes about healthcare are honored, and your choices about your assets are protected.
Clinic provides self-help assistance with the preparation of one of the following documents:
- Advance Healthcare Directive that explains how you want medical decisions about you to be made if you cannot make the decisions for yourself
- Power of Attorney that allows you to designate someone to act on your behalf in financial or legal matters
- California Statutory Will that allows you to leave your property or assets to your children or named heirs
This clinic is designed to help individuals understand and prepare these critical documents to secure their future and ensure their wishes are honored.
Registration is required. Document assistance is subject to capacity, income-level, and limited to persons living with HIV/AIDs and disabled veterans.
